Who we are
SlimLife Med is an independent sales organization that markets and routes inquiries to a US-licensed compounding pharmacy partner. We do not own the pharmacy. We do not take title to inventory. We do not store, dispense, or ship medications. All compounding, fulfillment, and prescription verification is performed by the partner pharmacy under its own state licensure and PCAB accreditation.

For licensed clinicians only
This site, the catalog, and all pricing information are intended exclusively for licensed clinicians, prescribers, and authorized representatives of licensed clinics. Nothing on this site is medical advice, a recommendation, or an offer of treatment to any individual patient. Patients should consult their own clinician.

Compounded medications
Compounded preparations referenced on this site are prepared by a third-party state-licensed and PCAB-accredited compounding pharmacy. Compounded medications are not FDA-approved and are dispensed only pursuant to a valid prescription for a specific patient. Pricing, availability, and acceptable substitutions are confirmed by the pharmacy at the time the script is processed.

Nothing on slimlifemed.com constitutes a claim of safety, efficacy, indication, dosing recommendation, or therapeutic equivalence. References to dosage strengths, vial sizes, and active ingredients reflect product specifications as supplied by the partner pharmacy and are intended to assist clinicians in catalog selection.

Pricing
Per-vial prices listed on the catalog reflect the operator’s published clinic rate at the time of listing. Prices are subject to change without notice based on partner pharmacy terms, raw material availability, regulatory changes, and other factors. Final pricing is confirmed inside the partner portal at the time a script is submitted.

Privacy
When you submit an application, we collect the information you provide (clinic name, your name, role, state, contact details, volume estimate) and use it solely to (a) verify eligibility, (b) initiate credentialing with the partner pharmacy, and (c) follow up about your application. We do not sell your information. We do not collect patient data on this site.

No PHI collected
SlimLife Med does not collect, store, transmit, or process patient protected health information (PHI). The site asks only for clinic-level business and licensure information from the prescriber or authorized representative. All patient-related data flows directly between the clinician and the partner pharmacy through the partner’s portal — never through us. We are not a HIPAA-covered entity, and clinicians should not transmit PHI to us via the application form, email, or any other channel.

Warranty disclaimer
The site, the catalog, all pricing information, and any related materials are provided "AS IS" and "AS AVAILABLE" without warranties of any kind, express or implied — including warranties of merchantability, fitness for a particular purpose, accuracy, or non-infringement. We do not warrant that listed SKUs will remain available, that pricing will not change, or that the partner pharmacy will accept any specific application.

Limitations of liability
Use of this site is at your own discretion. SlimLife Med does not guarantee uninterrupted access, error-free pricing, or specific outcomes. To the maximum extent permitted by law, SlimLife Med’s liability for any claim arising out of or related to this site is limited to the amount of fees you have paid us directly, which is typically zero — your commercial relationship is with the partner pharmacy. We are not liable for indirect, incidental, special, consequential, or punitive damages.

Indemnification
You agree to indemnify and hold harmless SlimLife Med, its operator, employees, and contractors from any claims, damages, or expenses arising out of (a) your misuse of the catalog or site, (b) submission of fraudulent or inaccurate information through the application form, (c) any practice of medicine you engage in based on information you obtained from this site, or (d) your violation of applicable state or federal regulations governing prescribing, compounding, or controlled substances.

Intellectual property
Site content, design, and copy are the property of SlimLife Med. SKU codes, product specifications, and tier designations referenced in the catalog are property of the partner pharmacy and used here under license for catalog-display purposes. Nothing on this site grants you a license to reproduce, redistribute, or republish site content without written permission.

Third-party processors
Application-form submissions are transmitted via Amazon Web Services (AWS Lambda + AWS SES) for delivery to our operator inbox. We do not run third-party tracking pixels, advertising trackers, or analytics that profile individual visitors. We may add a privacy-respecting analytics tool (such as Plausible) to measure aggregate page traffic; we will update this section if/when we do.

Modifications to these terms
We may update these terms from time to time. The "Last updated" date at the top of this page reflects the most recent change. Material changes will be highlighted at the top for at least 30 days. Continued use of the site after a change constitutes acceptance of the updated terms.

Governing law & disputes
These terms are governed by the laws of the State of Delaware, USA, without regard to conflict-of-law principles. Any dispute arising out of or related to these terms or your use of the site will be resolved exclusively in the state or federal courts located in Delaware. You waive any objection to venue or jurisdiction in those courts.
